I declare that I am the owner of all funds which I am going to invest in this Investment Agreement with Amicus Investment Ltd (AIL). I assure that the assets invested will not be used for illegal actions and that all information, which I have given in this application, is truthful. I will inform AIL if any information given will change in the future. I have read and agreed to AIL General Terms and Conditions and Special Conditions. AIL will repay me the invested capital at the maturity of the chosen account and the agreed return according to the terms of Amicus Investment Agreement. If the applicant is a minor there is needed the signatories of both parents (of all legal custodians). If there is only one legal custodian, a documentary proof for that should be attached.
